Medical Consent

Effective date: May 7, 2026

This document explains how care is delivered through Human+ and documents your informed consent to receive that care. By creating an account, completing an intake, or accepting a prescription through Human+, you acknowledge that you have read and understood this Medical Consent and that you agree to its terms.

1. Nature of telehealth

Telehealth is the delivery of healthcare services using electronic communications, in place of or in addition to in-person visits. Care through Human+ is delivered by independent, licensed U.S. medical providers using:

  • Asynchronous intake. You complete an online questionnaire about your health history, current medications, symptoms, and goals. Your provider reviews this material outside of a real-time visit.
  • Secure messaging. Your provider may follow up with questions, ask for additional information, or share guidance through the patient portal.
  • Synchronous video, where required. Some states and clinical situations require a real-time audio or video consult. Where required, you will be offered a scheduled visit before any prescription is issued.

2. Scope of services

Human+ supports evaluation and prescription, where clinically appropriate, of GLP-1 weight-loss treatment, including compounded semaglutide and tirzepatide dispensed by partner state-licensed pharmacies. Your provider may decline to prescribe and may recommend additional evaluation or in-person care. The decision to prescribe is a clinical judgment made by your provider; using the Services does not guarantee that you will receive a prescription.

3. Benefits of telehealth

  • Convenient access to care without travel to a clinic.
  • Faster review and follow-up than typical in-person scheduling.
  • A written record of your intake, your provider’s notes, and your medication history in one secure portal.
  • Adjustments to your plan in response to messages, without requiring a new appointment.

4. Risks and limitations of telehealth

As with any healthcare, telehealth has risks and limitations. These include, but are not limited to, the following:

  • Information you provide may be incomplete or inaccurate, which can affect clinical decisions. You agree to provide truthful, complete information.
  • Some clinical findings that would be apparent on a physical examination may not be detectable through telehealth. Your provider may need to refer you for in-person evaluation.
  • Technology can fail, messages can be delayed, and confidentiality cannot be perfectly guaranteed despite reasonable safeguards.
  • Telehealth is not appropriate for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency department.

5. GLP-1 specific risks

GLP-1 receptor agonist medications (including semaglutide and tirzepatide), and compounded versions thereof, may cause side effects. Common side effects include nausea, vomiting, diarrhea, constipation, abdominal pain, fatigue, and reduced appetite. Less common but serious risks include pancreatitis, gallbladder disease, severe gastrointestinal effects, kidney problems related to dehydration, low blood sugar (especially in patients on insulin or sulfonylureas), and allergic reactions. GLP-1 therapy is contraindicated for patients with personal or family history of medullary thyroid carcinoma, multiple endocrine neoplasia syndrome type 2, severe gastrointestinal disease, active pancreatitis, or who are pregnant or breastfeeding. Your provider will screen for these and other contraindications before prescribing.

Compounded GLP-1 medications, where prescribed, are prepared at a state-licensed 503A compounding pharmacy pursuant to your individual prescription. Compounded preparations are not themselves FDA-approved drug products. Your provider will discuss the differences between branded and compounded medications before prescribing.

6. Provider relationship and licensure

The medical providers who evaluate you through Human+ are independently licensed in your state. Human+ does not itself practice medicine. Your relationship for medical services is with your independent provider, not with Human+. Pharmacy services are provided by independent state-licensed pharmacies.

7. Confidentiality and records

Your medical information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws. The Notice of Privacy Practices on this site explains how your protected health information is used and shared, and your rights with respect to that information. Despite reasonable safeguards, no method of electronic transmission is perfectly secure.

8. Electronic prescribing and consent to communications

You consent to electronic prescribing of medication by your provider directly to a partner pharmacy. You also consent to receive electronic communications from Human+, your independent provider, and partner pharmacies, including by email, SMS, push notification, and in-portal message. Standard message and data rates may apply for SMS. You can opt out of marketing messages at any time; transactional and care-related messages will continue.

9. Right to withdraw consent

You have the right to withhold or withdraw your consent to telehealth at any time, without affecting your right to future care or treatment. Withdrawing consent will end the telehealth relationship; it will not retroactively affect care already provided.

10. Emergencies

Human+ is not appropriate for medical emergencies. If you are experiencing chest pain, difficulty breathing, severe pain, uncontrolled bleeding, an allergic reaction, suicidal thoughts, or any condition that could become rapidly life-threatening, call 911 or go to the nearest emergency department immediately.

11. State-specific addenda

Some states require additional disclosures, written consent, or an initial synchronous visit before prescribing. Where applicable, the additional disclosures and procedures will be provided to you during intake or in your patient portal as required by the state in which you are located.

12. Acknowledgment

By proceeding with care through Human+ I acknowledge that:

  • I have read and understood this Medical Consent.
  • I have had the opportunity to ask questions about telehealth and the proposed treatment.
  • The information I provide during intake is accurate and complete to the best of my knowledge.
  • I understand the risks, benefits, and alternatives to telehealth and to GLP-1 weight-loss treatment.
  • I agree to the terms of this Medical Consent.
